Welcome to the Integrated Care Program

The Illinois Department of Healthcare and Family Services (HFS) would like to welcome you as a member of the Integrated Care Program. The Integrated Care Program is for older adults and adults with disabilities ages 19 and older enrolled in an HFS Medical Program who live in: suburban Cook (Non 606 zip codes), DuPage, Kane, Kankakee, Lake and Will Counties.

This program promotes your health and wellness. Under the Integrated Care Program, a team of providers and case managers will work with you to help you reach the health goals that are important to you. In the program, you get to pick a health plan and a doctor or clinic as a Primary Care Provider (called PCP) for your medical home.

If you live in one of these counties – DuPage, Kane, Kankakee, Lake, Will, or suburban Cook (Non 606 zip codes) – and you meet the enrollment requirements, you can choose one of these two health plans:

        Aetna Better Health    OR    IlliniCare Health Plan

Please Note: If you are a member that is currently enrolled with a Primary Care Provider (called PCP) in Illinois Health Connect, you will be required to switch to a PCP in a health plan under the Integrated Care Program.

If your current Illinois Health Connect PCP is in one or both of the health plans that are being offered, you can choose to stay with that PCP.

If you live in Adams, Brown, Cook, Henry, Jackson, Kane, Lee, Madison, McHenry, Mercer, Perry, Pike, Randolph, Rock Island, Scott, St. Clair, Washington or Williamson counties, and have an HFS or All Kids medical card, visit the Illinois Client Enrollment Broker website at www.illinoisceb.com.

What is a Medical Home?

Your medical home is the doctor's office or clinic where you go to see your PCP. Having a medical home means:

What is a Primary Care Provider (PCP)?

What Services Will I Get in the Integrated Care Program?

You will get the same services you now get in Medicaid. The health plan you pick in the Integrated Care Program will work with you to coordinate and manage these services. Each health plan also offers extra services. Please use the Integrated Care Program Benefits Comparison Charts to see covered services and extra services that each health plan offers. You will not have to pay co-payments.

Do I have to enroll in the Integrated Care Program?

Older adults and adults with disabilities who are enrolled in Medicaid but not enrolled in Medicare and live in suburban Cook (Non 606 zip codes), Lake, Kane, DuPage, Will and Kankakee counties have to enroll in the Integrated Care Program.
